If you receive a narcotic/controlled-substance prescription VERBALLY from a person claiming to be Dr. Ricky Sheth, DO NOT DISPENSE.
If you receive a narcotic/controlled-substance prescription from a fax number OTHER THAN 289-870-2010, DO NOT DISPENSE.
Steps to Take If You Receive a Forgery:
1) Fax a copy of the forgery (or any verbal information collected) to Dr. Ricky Sheth’s ONLY fax number which is: 289-870-2010.
We must forward this information to the Ontario Provincial Police.
2) Contact your local police department’s non-emergency phone number to report the forgery.
PLEASE DO NOT WAIT FOR CONFIRMATION FROM US.
The prescription is by DEFAULT a confirmed forgery if::
a) You received a verbal prescription for a controlled-substance;
-OR-
b) The letterhead of the prescription has any fax number OTHER THAN 289-870-2010.
This is Dr. Sheth’s only fax number.
